2011-2012 Liability Release Form

This form is a liability and release form for any event occuring from September 1, 2011 through August 31, 2012. It requires more information and consent to the Student Discipleship Handbook Policies and Guidelines.

Event Liability Form
This is a form for a single event. It requires less information and does not require consent to the Student Discipleship Handbook. However, this form is only good for a one event.
